Primary care is the foundation of ongoing healthcare. At Core Care Clinic, our primary care team supports preventive care, routine visits and everyday health concerns while helping patients navigate longer-term health needs.
What Is Primary Care?
Primary care can be a starting point for routine checkups, preventive care and many everyday health concerns. Your clinician can review your health history, discuss concerns and help determine appropriate next steps.
Primary Care Services
Our primary care services can include routine and preventive visits, health screenings, care for common health concerns and ongoing management of certain chronic conditions. The right care depends on your individual needs.
1
Preventive Care & Routine Visits
Preventive care helps patients stay current with recommended health checks and discuss changes in their health. Your clinician can review your history and recommend appropriate screening or follow-up based on your needs.
2
Care for Common Health Concerns
If something does not feel right, primary care can be a starting point for evaluation and guidance. Your clinician can discuss your symptoms and health history and determine whether testing, follow-up or specialist care may be appropriate.
3
Chronic Condition Management
For patients living with an ongoing condition, primary care can support monitoring, follow-up and coordination of care. The care plan should be based on the patient's individual health needs.
4
Care Coordination & Referrals
When care outside primary care is needed, coordination can be an important part of the patient journey. Your primary care team can help determine when additional evaluation or specialist care may be appropriate.
Why Choose Us
Choosing a primary care team you can rely on year after year makes a real difference – not just in convenience, but in the quality of care you actually receive.
One connected team: Your primary care and behavioral health providers work together, not in separate silos you have to coordinate yourself
Real continuity: The same provider tracks your health over time, catching subtle changes a one-off visit would miss
Access when you need it: Same-day appointments and telehealth mean you're not waiting weeks for care that can't wait

FAQ’s about the Service
What does a primary care doctor do?
A primary care doctor is your main point of contact for ongoing health needs - from annual physicals and preventive screenings to managing chronic conditions and treating everyday illness. They get to know your health history over time, which means better continuity of care than seeing a different provider every visit.
What should I bring to my first appointment?
A list of current medications and supplements, your insurance card and photo ID, any recent records from other providers, and a general sense of your family health history if this is a new relationship with your provider. Writing down any questions beforehand helps make sure nothing gets forgotten once you're in the room.
Can I book a routine physical?
Yes - annual physicals and wellness visits are available at all three of our locations. These visits are a chance to review your overall health, catch up on recommended screenings, and discuss anything that's been on your mind, even if it doesn't feel urgent enough for its own appointment.
When should I contact primary care about a concern?
If something doesn't feel right - a new symptom, a nagging concern, or a change you've noticed - it's worth reaching out rather than waiting. Primary care is a good starting point for evaluation and guidance, and same-day appointments are available for concerns that need attention now.
Does Core Care accept new patients?
Yes, we're currently accepting new adult patients at all three of our locations - Waltham, North Andover, and Tampa.
Are same-day appointments available?
Yes. Same-day appointments are available for urgent, non-emergency needs like infections, minor injuries, or sudden symptoms - so you're not stuck waiting weeks when something needs attention now.
